To make sure we have identified the pole that needs repair, we need the following information:
- Pole number (The number is located 6-10 feet up from the bottom of the pole)
- Closest street address near the pole
- Closest cross street near the pole
- Your name and daytime phone number (in case we need to follow up with you)
